The Muskoka Community Health Link is a partnership between the District Municipality of Muskoka, the Algonquin and Cottage Country Family Health Teams, the North Muskoka Nurse Practitioner-Led Clinic, the North Simcoe Muskoka Community Care Access Centre, Muskoka Algonquin Healthcare, Canadian Mental Health Association, Muskoka- Parry Sound Branch and other community agencies.
Muskoka’s Community Health Link - one of more than 80 Health Links that have been created by the Ministry of Health and Long-Term Care across Ontario. Community Health Links are an opportunity for care providers to work together to make sure patients and their families experience better, more coordinated care
